Preparing for an Appointment

Preventive Screening Exam:

  • Morning Appointment: In the event our healthcare provider chooses to order laboratory studies, we ask that you come to your appointment fasting. This means no food 12 hours prior to your appointment time. You may drink water and you may take your scheduled medications as long as you are not diabetic. Please no powder or lotion on your body, deodorant is fine.
  • Afternoon Appointment: You may take your scheduled medications. Please no powder or lotion on your body, deodorant is fine.

Pre-Operative Exam:

The purpose of this examination is to clear you for a surgical procedure. It is imperative that we have the orders from the surgeon. Although the surgeon’s office may fax the orders to our office, we request that you also bring a copy with you to the appointment. Without the needed orders, we will not be able to complete the examination.

First Time Appointments:

Thank you for choosing our practice. In an effort to make your transition smooth, we request that you bring a list of all of your current medications and the names of any specialists that you are currently seeing. Please complete the health history questionnaire we provide or kindly provide your own list.
